Because we know that beginning treatment with a new physician can be confusing and time-consuming, we’ve prepared the following guide to help you get in, get seen, and get better – on your own schedule.
What to Bring With You to Your Appointment
- A photo I.D.
- Your insurance card.
- A list of current medications.
- The names and contact information for your other physicians.
- Any pertinent medical records relating to the subject of your visit.
- The shoes you wear most often.

Scheduling and Cancellation Policies
We make every effort to accommodate the scheduling needs of our new patients. We have convenient evening, Saturday, and same-day appointments available. Because we understand that most patients are experiencing some pain by the time we have an opportunity to see them, we’ve created an atmosphere that’s relaxing, calm, and completely focused on your needs. Additionally, unlike many physician offices, we do not schedule more patients than we can see within a reasonable time, which means that your days of passing an hour or more in a waiting room are things of the past.
However, as a courtesy to other patients, we do request that you call to cancel your appointment within 24 hours prior to your scheduled appointment time.
When to Arrive for Your Appointment
We know all too well that sometimes things just don’t run on schedule. We do ask you, though, to do your best to arrive on time. To ensure that you don’t spend more time in our waiting room than necessary, we’ve supplied you with the forms we use to collect all of the information needed to begin crafting your individualized treatment plan. But if you don’t have the time to fill out these forms at home, please try to arrive 10 minutes in advance.
